What is oxiracetam?
Oxiracetam is a synthetic hydroxylated racetam studied for cognitive impairment. It is not a vitamin, mineral, herb or dietary requirement. Proposed mechanisms include cholinergic transmission, neuronal membranes and glutamatergic signaling, but mechanism alone cannot establish useful cognition or daily function. [6]
The evidence has a clear modern anchor. A government-commissioned, 30-center South Korean trial randomized 500 people at high risk of post-stroke cognitive impairment. After 36 weeks, oxiracetam did not improve either co-primary outcome. That result is more informative for current claims than small old trials or healthy-user anecdotes. [1]
Older dementia trials reported benefits on selected cognitive or quality-of-life measures. They explain continuing research interest, but none tested healthy students or workers seeking above-baseline performance. [2]–[4]
Typical Oxiracetam Research Protocols
| Study and population | Product and route | Amount each administration | Daily total | Frequency and duration | Main interpretation |
|---|---|---|---|---|---|
| Gschwind 1992 · human radiolabeled disposition experiment | Oral 14C-labeled oxiracetam | 800 mg | 800 mg | Single administration | Pharmacokinetics only; no cognitive endpoint |
| Maina 1989 · 307 enrolled with degenerative, multi-infarct or mixed dementia | Oral 800 mg tablet | 800 mg | 1,600 mg/day | Twice daily for 12 weeks | Three older efficacy scales favored oxiracetam; 289 analyzed |
| Bottini 1992 · 65 enrolled with mixed dementia types | Oral 800 mg tablet | 800 mg | 1,600 mg/day | Twice daily for 12 weeks | Selected tests and quality-of-life scale favored oxiracetam; 58 completed |
| Lim 2026 · 500 adults at high risk of post-stroke cognitive impairment | Oral oxiracetam | 800 mg | 1,600 mg/day | Twice daily for 36 weeks | No benefit on either co-primary endpoint; 457 completed |
The repeated 800 mg amount reflects specific medicine trials, not a universal nootropic target. In Lim, it was the standard approved regimen in South Korea and was tested specifically because regulators were reassessing a legacy drug. The negative result does not become positive by selecting a more active subgroup: the treatment-by-activity interaction was nonsignificant. [1]

What is oxiracetam commonly used for?
Post-stroke cognitive decline
Lim and colleagues enrolled 500 adults aged 50 or older, at least three months after stroke, with subjective cognitive decline and additional risk features. Oxiracetam 800 mg twice daily for 36 weeks did not differ from placebo for MMSE change (+0.13 versus +0.27; p=0.49) or CDR-SB (−0.14 versus −0.08; p=0.38). Physical activity did not significantly modify treatment effect. [1]
The highly active subgroup showed exploratory trends, but the formal interaction was null. That distinction prevents a post hoc trend from becoming a treatment rule.
Dementia and older cognitive-impairment trials
Maina randomized 307 people with primary degenerative, multi-infarct or mixed dementia; 289 were analyzed and 272 completed. Three main older scales favored 800 mg twice daily over placebo after 12 weeks. Minor unwanted effects were similar between groups. [3]
Bottini enrolled 65 people with similar mixed dementia diagnoses and reported better quality-of-life and selected neuropsychological results; 58 completed. Villardita’s 40-person study also reported improvements on selected tests. These are genuine positive records, but they are small or old, use heterogeneous diagnoses and scales, and do not erase the modern co-primary null result. [2], [4]
Healthy cognition
No strong controlled trial establishes better focus, recall or study performance in healthy young adults. Animal models, mechanisms and impaired populations can generate hypotheses, but they cannot prove an above-baseline nootropic effect.
Evidence over time
Older signals meet a modern null trial
What happened in the negative organic-brain-syndrome trial?
Hjorther studied 106 middle-aged people with cognitive symptoms attributed to prolonged organic-solvent exposure for 12 weeks. No neuropsychological, symptom or global measure differed from placebo. The indexed abstract reports “2.4 mg per day,” an implausible unit; this guide does not silently convert it to grams or use it as a protocol amount. [5]
How do forms and labels differ?
Oxiracetam is chiral and is usually discussed as a racemic compound; isolated S-oxiracetam animal data do not establish equivalence for a retail racemate. A dispensed medicine, imported tablet, online capsule and bulk powder also differ in authorization, manufacturing controls and traceability.
Laboratory studies of products labeled with unapproved cognitive drugs have found inaccurate amounts and undeclared compounds. A certificate or “research use” label cannot substitute for an authorized, traceable finished product. [7]
How should oxiracetam be stored?
Follow the authorized pharmacy label where applicable. Keep tablets or capsules in the original labeled container, dry and inaccessible to children. Do not store unidentified powder beside food or routine supplements. Storage cannot correct substitution or inaccurate strength.
What does the human evidence show?

| Question | Best direct evidence | Finding | Boundary |
|---|---|---|---|
| Does oxiracetam prevent post-stroke cognitive decline? | 500-person, 36-week randomized trial | No benefit on MMSE or CDR-SB | High-risk post-stroke population; physical activity interaction null |
| Did older dementia trials find benefit? | 307-person and 65-person 12-week trials | Selected scales favored oxiracetam | Older mixed diagnoses and outcome instruments |
| Does it help solvent-associated organic brain syndrome? | 106-person randomized trial | No significant difference | Source reports an unresolved dose-unit anomaly |
| Does it enhance healthy cognition? | No strong direct trial base | Unproven | Impaired populations cannot be transferred |
| Are online products equivalent to trial medicine? | Product laboratory analyses | Identity may be unreliable | Sampled products do not estimate the whole market |

What are the risks and interactions?
In the Lim safety population, any adverse event occurred in 100 of 244 oxiracetam participants (41.0%) and 88 of 252 placebo participants (34.9%); p=0.16. Serious events occurred in 21 and 16 participants, respectively; p=0.34. Investigators found no clinically relevant attributable signal, but one trial cannot exclude uncommon harms. [1]
Older trials reported sleep disturbance, psychomotor excitability, GI symptoms and tolerability withdrawals. Bottini recorded two oxiracetam withdrawals for poor tolerability and one transient ischemic attack judged unrelated. Maina recorded three poor-tolerability withdrawals in each group. [2], [3]
Because oxiracetam has central nervous system activity and is cleared through the kidneys, review stimulant, sedative, seizure, cognitive and renal contexts with a clinician. Do not infer that choline prevents headaches or makes a gray-market product safer. Seek urgent care for seizure, severe agitation or confusion, chest pain, fainting, allergic symptoms or sudden neurologic change.
What is the legal and regulatory status?
In the United States, FDA has named oxiracetam in a warning letter addressing unauthorized drug claims and stated that the listed compounds do not qualify as dietary supplements or conventional foods. Australia names oxiracetam under the Schedule 4 racetam entry, which is prescription-only. These facts do not authorize any online seller. [8], [9]
In Canada, verify any claimed medicine through the Drug Product Database and any claimed natural health product through the Licensed Natural Health Products Database. An online listing is not evidence of a DIN or NPN. [10]
For athletes, absence by name from a list is not clearance. WADA S0 can apply to pharmacological substances without current governmental approval for human therapeutic use. Check current rules with the governing body and do not use Global DRO as a finished-supplement certification tool.
When should use stop?
Stop an unverified product and seek advice for new insomnia, marked agitation, mood change, persistent GI symptoms or any neurologic worsening. Do not start or stop a prescribed cognitive medicine without the prescriber.
What should be monitored?
In medical care, record the specific cognitive and functional endpoint before treatment, baseline status, administration times, adherence, sleep, adverse effects and co-medication changes. Review whether any observed change exceeds ordinary test variation or recovery after stroke.
The Lim trial illustrates why a prespecified endpoint matters: exploratory subgroup trends did not change the null treatment-by-activity interaction or the two null co-primary outcomes.
What does community experience add?
Public reports may surface sleep disruption, headaches, mood changes, timing and sourcing problems. They cannot establish product identity, a universal choline rule or reliable focus benefit. No systematic current community sample was assembled, so this guide reports no consensus healthy-user dose, cycle or response rate.
How does oxiracetam compare with related options?
| Compound | Evidence focus | Main distinction |
|---|---|---|
| Oxiracetam | Modern post-stroke null trial and older dementia studies | Strong contemporary negative anchor; healthy enhancement unproven |
| Piracetam | Memory synthesis and specialist myoclonus | Broader clinical history and different neurologic indication |
| Aniracetam | Old dementia study and scopolamine challenge | More lipophilic AMPA-modulating racetam; no routine healthy evidence |
| Phenylpiracetam | Sparse performance evidence and athlete concern | Different molecule and anti-doping context |
Common questions
Is oxiracetam a supplement?
It is a synthetic racetam with medicine-like use and regulation in some jurisdictions. A supplement-style label does not make it a nutrient or prove legality.
Did the newest trial work?
No. The 500-person trial found no benefit on MMSE or CDR-SB after 36 weeks. [1]
Did active participants benefit?
Exploratory trends appeared in a highly active subgroup, but the formal treatment-by-activity interaction was not significant. That cannot support an activity-specific recommendation. [1]
Why do older trials look more positive?
They studied different dementia populations, used older scales and were much smaller or shorter. Differences in era and design can change results.
Is 800 mg twice daily a typical dose?
It is a source-specific medicine schedule used in several trials. The modern trial was negative, so repetition in research does not make it a validated consumer dose.
Is the 2.4 mg/day figure from the Hjorther abstract reliable?
The indexed abstract prints that unit, but it is implausible and cannot be safely corrected without a verified primary method. This guide excludes it from the protocol table. [5]
Does oxiracetam require choline?
No controlled evidence establishes a universal requirement. Adding another active product can add effects and obscure attribution.
Is it allowed in sport?
Do not infer clearance from omission by name. The WADA S0 non-approved-substance rule and product contamination both require current specialist review.

Editorial note
This guide lets the modern null post-stroke trial lead while retaining older positive and negative studies. It does not turn a repeated medical-trial schedule into a consumer recommendation.
